Skill Development

We provide hands-on training in self-care, money management, and socialization skills tailored to each person’s goals. Staff coach daily routines like hygiene, dressing, meal planning, medication prompts, and housekeeping so participants gain confidence at home. We build practical money skills, creating a simple budget, comparing prices, shopping with a list, paying bills, and using cash or a debit card safely. Social goals focus on communication, friendships, community activities, and healthy boundaries, with plenty of real-world practice in the neighborhood. All instruction is paced to the individual and supported in their preferred language.

Behavioral Support

We provide respectful assistance and coaching to support positive, sustainable behavior change. Starting with a strengths-based assessment, we collaborate with the individual, family, and case manager to identify goals, triggers, and what already works. Staff teach and practice replacement skills, clear communication, asking for help, using visual schedules, coping and calming strategies, and problem-solving while reinforcing successes in real settings (home, neighborhood, appointments). We prioritize proactive supports over consequences, use de-escalation and safety plans when needed, and track progress discreetly so we can fine-tune the plan. Coaching is trauma-informed, person-centered, and delivered in the individual’s preferred language to protect dignity and choice.

Personal Assistance

We provide respectful, hands-on assistance with daily tasks and personal care so each person can live safely and comfortably. Trained staff support bathing, grooming, dressing, toileting, mobility, and meal preparation, as well as light housekeeping, laundry, and room organization. Help is offered at the least intrusive level prompts and coaching first, hands-on support when needed to protect dignity, privacy, and choice. We also assist with medication self-administration reminders, adaptive equipment, and simple routines that build independence over time, documenting progress and adjusting supports to the individual’s preferences and goals.

Family Coaching

We partner with families to reinforce skills and promote independence. During visits and scheduled check-ins, we coach practical strategies, prompting, and fading, visual schedules, communication supports, and safety routines so goals stick at home and in the community. Together we create a simple home plan with step-by-step checklists, progress tracking, and positive reinforcement ideas, and we provide plain-language, multilingual handouts. As skills grow, we intentionally step back, transferring more responsibility to the individual and caregivers while staying available for tune-ups and problem-solving to keep progress moving forward.

My brother moved into a Gem State home this spring, and the difference is incredible. He has structure without losing independence daily routines, community outings, and respectful support 24/7. The team is organized, kind, and transparent about goals and safety plans. They call back fast and actually follow through. We trust them completely.
